The Request Template, Recall Template and Approval Template lookup fields return blank values. Have I missed something in the configuration?

These fields need to be populated on the Approval Rule object. They don't get populated automatically.

I am currently working on my first CPQ implementation and I'm astonished at how much extra configuration is needed to get this product working as per some of the demonstrations that I have seen.
It is most definitely not a seamless integration and out of the box solution.
Is there a description somewhere that describes how to populate the fields to the approval rule object?
I can't find an object called "Approval Rule"
